Course Title: Spiritual Exercises.
Venue: Name of the school.
Trainer: Name of the Spiritual Exercises animator.
Address: Address of the animator.
Month: The month when the spiritual exercises were done.
Tel / Mobile: number of telephone or mobile.
ID: ID card number.
NI: National Insurance or Social Security (SS) is on your payslip.
VAT: Not applicable.
IBAN no: The animator’s bank account number. Preferably, type it. If written, write it very clearly.
Signature: The animator’s signature.
Date: The date when the form was completed.
In the form, provide the information about the whole Spiritual Exercises course, including the sessions AND the celebration of the sacrament of reconciliation and the Eucharist.
You shall send the remuneration form for Spiritual Exercises by Friday, 16th May 2025.
